Your intake form — PS NOW

Welcome — and thank you for saying yes. This is the one thing I genuinely need from you before we start.

To tailor your eight weeks and to keep you safe, I have to ask about your health and your experience with the medication. Some of that is what the law calls special category data, so I only collect it with your explicit permission. Your answers are stored in Belgium, on Tally, the platform that runs this form — the answers themselves go nowhere else, and I read them myself. The one thing that does travel is the result: which focus area and starting point I assign you, so your course area can show you your own materials. The last page explains that in full.

It takes about 12 to 15 minutes. Most of it is ticking boxes. The last part has five open questions and only one of them is required — so if you're short on time, answer that one and move on.

You can leave this one unticked. It changes nothing about your place or your programme.

I consent to my anonymised answers being used to improve the programme and for aggregate insights.

You can withdraw your consent at any time by emailing support@ps-now.com — that won't affect anything already processed. Everything is deleted within six months of the programme ending.
